The xray below is of a 2 year old male who underwent a redo of their HD pull-through for transition zone. The right colon remained and this was pulled through down the right pelvis. They started stooling well on postoperative day #3 and were started on a diet. On postoperative day #5 they began to have abdominal distension and emesis. Based on these symptoms and this xray what do you think is the problem and what would you do?
Answer for April 24th
Notice the opacification in the pelvis. How might you explain that? It likely could be a distended bladder which is causing the ileus. I would check a post void residual, and consider placing a foley catheter which might improve the GI symptoms.
