Here is the cloacagram from an 8 month old baby girl. She had a tethered cord which was detethered. What would be your approach? Posterior sagittal only? Total urogenital mobilization? Urogenital separation? How long would you leave the foley in? Would you use a suprapubic tube?

This is a cloaca with a low confluence, 2 cm common channel and 2 cm urethra, so total urogenital mobilization is the right approach. All can be completed posterior sagittally. In such a case I would place an SP tube for a planned clamping trial after foley removal, because given the spine status the patient might not void spontaneously and might need to have intermittent catheterization to ensure complete bladder emptying.
