Last week I asked you about a 12 year old female with lifelong constipation managed with a stool softener who presented with massive abdominal distension, peritoneal findings, and the below xray. She was taken urgently to the operating room and was found to have a sigmoid volvulus.
The surgeons who cared for her untwisted the volvulus, the sigmoid was viable, and decided to perform a sigmoid resection with primary anastomosis. On POD #5 the patient had a leak from that anastomosis, a tiny hole at the staple line, and she underwent re-exploration, and performed a colostomy with a Hartman’s pouch.
How you handled the initial case varied, some of you tried a sigmoidoscopic reduction, some detorsed the volvulus and left the sigmoid in place, some did a resection with anastomosis, some did a stoma, some did a rectal biopsy.
I think I might have done a sigmoid resection up front to avoid another torsion and would have sent a rectal biopsy. It’s a tough case with difficult decision making in the middle of the night.
Now for this week, we now have this patient with a colostomy and a Hartman’s, recovered from the operation and doing well.
