5-year-old female with ARM, who underwent a Malone to offer antegrade flushes. After tolerating a regular diet, the flush given was 400 cc saline plus 20 cc glycerin. Several hours later she vomited and got distended, and here is here x-ray. What do you think the problem is? What would you do?

At first glanced you might think this emesis after the flush was related to the flush itself, most specifically the added glycerin. However, look closely at the xray. You’ll note an opacification in the pelvis. This patient had a bladder scan and had 280 cc in their bladder – i.e. urinary retention. A foley was placed and the nausea/vomiting resolved and she tolerated the flush the next time without difficulty. A void trial conducted two weeks later was successful and there was no need for intermittent catheterization.

A 9-year-old born with covered cloacal exstrophy has been living her life with an ileostomy, which was done in the newborn period. She has been growing well. She leaks urine out the perineum at all times. The bladder neck is wide open on cystoscopy. The single perineal opening (the introitus) is good size and two cervices are visible. There is no anal opening, but she has good sphincteric contractions. A contrast study is shown below.
