Baby girl with ARM, rectovestibular fistula, who had a spinal ultrasound in the newborn period that showed tethered cord, and nothing else. She underwent an uneventful PSARP. At age 6 months she underwent spinal MRI to plan for a tethered cord release, which is shown here.
This is a presacral mass, found incidentally during the MRI for the spine to assess for tethered cord. It was not seen on the newborn spinal ultrasound. This is most likely a teratoma and needs a resection, via a posterior sagittal approach. The anus has already been managed during the PSARP done at another institution. Neurosurgery chose to have this mass removed as a separate surgery from their planned tethered cord release.
