Colorectal Conundrum for October 30th:
A 2 year old boy with HD, who underwent a laparoscopic assisted Swenson pull-through in the first weeks of life presents with constipation. In the office you detect an anastomotic stricture. What would be your plan?
1. Dilate in the office digitally and with Hagar dilators, then have family continue to dilate at home
2. Dilate in the OR digitally and with Hagar dilators, then have family continue to dilate at home
3. Dilate in the OR digitally and with Hagar dilators and give kenolog (steroid) and have the patient return for another OR exam in one month, no intervening dilations at home
4. Dilate in the OR digitally and with Hagar dilators and give mitomycin and have the patient return for another OR exam in one month, no intervening dilations at home
Answer for October 30th:
There is no good answer to this question – there is quite a lot of variation to the treatment in this situation. I have been having the family dilate with a Hagar after detecting a stricture, which I like to dilate up in the operating room and give steroid (kenolog). They can then pass a dilator thereafter and then I would recheck under anesthesia at one month.
