Last week, I asked you about the below urethral problem in a patient with a prior PSARP for a rectoperineal fistula and an intraoperative urethral injury. A vesicostomy was ultimately created and they were referred to us for urethral repair.
Our research fellow, Liza Bokova, came up with a stellar response. She wrote:
“The first step in managing a urethral injury is a urinary diversion with an SPT/vesicostomy and urinary stent placement. To assess the anatomy and define further operative plan, cystoscopy and a contrast study (RUG) are needed.
This patient has a urethral stricture and requires a redo PSARP with urethroplasty. If the length of the urethra is enough to approximate the urethral ends without tension, a primary urethral anastomosis can be otherwise, either buccal mucosa or a rectal patch can be used as a graft dependent on the length of the defect. After the reconstruction, a circular urethral stent and vesicostomy are kept in place. In 4-6 weeks (at the time of colostomy closure), the postoperative anatomy is assessed with a cystoscopy and voiding trials.
We approached the urethra with re-mobilization of the rectum, which gives a great view of the urethra and bladder neck. Our urology team was able to perform a primary urethral repair.
For this week – a not uncommon problem with a cute trick to solve it.
In clinic, a patient one month after their Malone procedure was here to learn cathing of the tract. Our APP cut the Malone catheter suture and deflated the balloon (we had placed a 10F coude foley in the tract at the time of Malone creation). But, to their surprise, they could not remove the foley. It was stuck.
