Last week I asked you about a patient with spina bifida who needs a Malone and Mitrofanoff. The colorectal and urologic teams are working together and are deciding whether the appendix can be shared. I showed you the below photo and asked what options to consider:
This is a normal cecum – (some of you were concerned about the bulgy part). To make this decision please see the below diagram created by our good friend Devin Halleran, who is about to become the pediatric surgery fellow in Portland, Oregon. The mitrofanoff is best served by using the appendix, so all efforts should be made to use the appendix for that purpose, but it needs to be at least 5 cm long. If it is shorter than that the appendix can be the Malone and the Mitrofanoff made from a piece of small bowel (Monti). If the appendix is 7 cm or greater it can be split, and if the appendix is between 5 and 7, it should go for the Mitrofanoff and a neoMalone created from a flap of right colon.

This doesn’t seem like great sharing, and I joke with our urologists that their version of sharing is well depicted in the cartoon below. It is clear thought that long term the appendix is the best option for the Mitrofanoff, and a Malone or neoMalone have similar results for the antegrade colonic access. So the urologists get to choose!
For this week we have quite the challenge. The patient in the photo below is a male born with imperforate anus who underwent a diverting loop stoma. The stoma has obviously prolapsed, and is not reducible. It is pink and viable and produce stool. In the radiology department a distal colostogram could not be done because of this prolapse. How would you proceed?
