Good morning!
Last week, I asked you about the following patient sent to us from our friends at Montreal Children’s. This newborn baby with concern for HD had the contrast study shown below.
This patient clearly will need a biopsy given HD symptomatology. Interestingly on the contrast study, the very distal aspect of the rectum seems abnormal.
For where to biopsy – we got a wonderful answer from my friend, Shannon Castle, who works at Valley Children’s Hospital in California. She replied:
“Bedside suction biopsy. 3 specimens. 1 and 2 cm. Then I thank the good Lord for pediatric pathologists.”
I like that answer a lot. 1 cm just above the crypts should be a good spot and 2 cm to further confirm. The pitfalls are biopsying too low (in the anal canal) which is always aganglionic, but also biopsying too high – and missing a low transition zone. Suction biopsy is safe and reliable in those under age 1, if the suction biopsy is unclear then an open biopsy is needed.
