Based on this new image – here is the distal colostogram with inadequate pressure and the properly done study side by side. What is the malformation and what would be your approach?

It’s amazing how different are the two images! The first study was done without adequate pressure. The second with adequate pressure which clearly shows a very low rectum – no fistula seen. The approach is PSARP, and opening the posterior wall of the rectum, making sure there is no low bulbar fistula and mobilizing the rectum or the anoplasty.
