Last week, I asked you about a case a newborn who was taken to the OR for distension and an upper GI that which was read as malrotation. No malrotation or volvulus was found, just a dilated colon. A rectal biopsy was done and came back as HD. In the NICU thereafter the patient developed free air and was explored. A cecal perforation was identified and an ileostomy created with repair of the cecum. The patient comes to you with an ileostomy, and confirmed HD by rectal biopsy. I asked what would your plan be.

Most of you chose to level this patient with biopsies of the colon and to wait for permanent section. I agree. The contrast study shows that possible the right colon and the right transverse colon are good – but proximal to the splenic flexure I always will wait for permanent section and will not do a primary pull-through based on frozen section. Remember proximal to the left colon you cannot expect to see hypertrophic nerves (there is no sacral plexus innervating that bowel) so you are completely dependent upon your pathologist seeing ganglion cells on frozen section. If they don’t see them you risk taking out good bowel that was incorrectly deemed aganglionic.
For this week, what would you do with this ARM female patient who underwent initial repair (after a newborn colostomy) for what was likely a rectovestibular fistula. After the colostomy was closed she passed stool via the vagina. Below is a contrast study showing the fistula and the distal rectum. Cleary she needs a redo, but let’s be specific with a plan.
Diversion now then repair?
Repair plus diversion?
No diversion?
